Abstract: 

Introduction: The right use of blood is necessary because of the high cost of correct blood screening, blood grouping, and cross match. The purpose of this study is to evaluate the rate of the reserved and cross matched blood before elective surgeries which were done in Bahman 22nd Hospital. It also aims at evaluating the standard guideline for ordering blood.Materials and Methods: This is a retrospective study from 1380 to 1388 that evaluates evaluating 6145 files of the patients hospitalized in 22 Bahman 22nd Hospital for surgery during the years of 1380 to 1388. The elective operations include General surgery, Gynecology, orthopedic, ENT, Urology and Neurosurgery. The number of patients, the number of reserved and cross matched units and the number of transfused units was recorded for each group of patients collected in each groups. Finally, the number of reserved blood units in that period of time and in each group was compared with the standard guideline. Also the number of transfused blood units was evaluated. The ratio of cross matched to transfused blood units lower than 2.5 was considered as standard. And The obtained data were analyzed, using with SPSS.Results: A total of 1557 blood units of blood were reserved for 772 patients.1305 units (66.47%) were cross matched, but just 126 units (8.1%) were transfused. The effective factors in increasing the possibility of transfusion were the ages<20 & >80 years old, Hb £ 12, the type of surgery and the surgeon. Although after the application of the guidelines in1385, there was 64.44% decrease in the rate of cross matching rate due to the decrease in the number of patients who were cross matched, but the average of cross matched units for each patient did not change. The total of C/T index of the hospital decreased from 14.1 to 3.7.Conclusion: The appropriate correct blood reserving can decrease the unnecessary blood reserves, lower the pressure on blood banking centers and prevent blood wastage.

Abstract: 

Amount of fumonisin B1 in 46 corn samples which were collected preharvest, at harvest and post harvest stages (after drying, befor and after silage) from Golestan province in northern Iran was evaluated. The samples were milled using analytical Romer mill. Fumonisin B1 was extracted with methanol-water (80:20 v/v) solvent, purified by immunoaffmity columns (IACs) and converted to fluorescent derivatives by OPA (Ortho-phetaldehyde).The mycotoxin was quantified through high-performance liquid chromatography (HPLC) with fluorescence detector (Em=335 nm, Exi=440 nm). Standard curve was drawn between 0.3125-40 mg/ml using 6 fumonisin working standards. Validation of the method was estimated by Certified Reference Material (CRM). The percentage mean of recovery obtained using this method was 90.7 %. All samples were contaminated with fumonisin B1, the range of contamination varied from 261 to 6891 ng/g and the mean of contamination was 2658.35 ng/g. There was in significant difference on the contamination of samples at different sampling stages.

Abstract: 

BACKGROUND: FMD is one of the most important animal health problems in the world and is ranked at the top of the list A of potentially epidemic infectious diseases of livestock (OIE). FMD virus infects a wide range of domestic and wild cloven hooved animals and causes clinical signs. The disease is mild zoonotic and 70 wild mammal species from 20 animal families are susceptible to infection. Also, birds are mentioned as transferring agent of FMD virus in several references. OBJECTIVES: The motivation of this study was due to observation of a significant presence of pigeons in FMD involved farms in the epidemic of serotype O2016 in the first months of 2016. METHODS: After hunting of six pigeons gathering food in FMD involved farms, their blood samples were collected. In the laboratory, FMDV genome was traced by RT-PCR with aphtovirus universal primers and final product was sequenced. RESULTS: The 328 bp band indicating a positive result was observed in electrophoresis of all samples. These results were also confirmed in repeated experiments. Then the RT-PCR products were sequenced in both directions. Alignment and BLAST results indicated more than 97% identity of virus from samples with FMD registered viruses in Genebank, demonstrating the presence of FMD virus genome in the blood of the pigeons. CONCLUSIONS: This result indicates FMD virus genome viremia in the blood of pigeons. It is worth noting that pigeons’ infection is very important because this species is a free flight bird and has the possibility of transmitting the virus over long distances, thereby causing new epidemics. Finally, it is necessary for further studies to investigate the possible presence of clinical signs in the pigeons, the possibility of shedding, routs and virus titers of shedding from any of the possible ways.

Abstract: 

Background and Objectives: Umbilical cord blood has been used successfully as a source of hematopoietic stem cells for transplantation. Knowing that cord blood could be bacterially contaminated at the time it is being obtained and afterwards during the time it is being processed, we decided to perform the present study to determine any possible bacterial contamination and find out the root causes in Royan Cord blood Bank (RCBB).Materials and Methods: RCBB during 3 years received 3074 cord blood units (CBUs) upon which relevant investigation were made. In order to determine the root cause of the bacterial contamination, 800 CBUs were tested out of each of which two samples were taken. The data were analyzed with Chi-square by SPSS 16.Results: Out of 3074 CBUs, 93 units (3.02%, CI 95%=2.3-3.6) were shown to have bacterial contamination. Out of the 800 units tested to detect the root cause, 26 samples (3.3%) were proved to be bacterial culture positive out of which 25 (96.2%) were shown to be contaminated at the time they were obtained and 1 (3.8%) during processing. The isolated bacteria were aerobic in 19 cases (73.1%) and anaerobic in 7 (26.9%).Conclusions: The results show that bacterial contamination mostly is caused at the time blood is obtained from the cord in hospital obstetrics wards and most of the isolated bacteria were shown to be skin flora. Given the high value of cord blood stem cells and the risk of septic transplantation, it is necessary to prepare training programs for midwives and phlebotomists.

Abstract: 

Statement of the Problem: In clinical situations, Calcium-Enriched Mixture (CEM) comes into direct contact or even mixes with blood during or after placement.Purpose: The aim of this study was to evaluate the effect of blood contamination on the compressive strength of CEM.Materials and Method: Three experimental groups were included in this study. In the first group, CEM was mixed with distilled water and was exposed to normal saline (control group). In the second group, CEM cement was mixed with distilled water and then was exposed to blood. In the third group, CEM was mixed with and exposed to blood. Nine custom-made two-part split Plexiglas molds with five holes were used to form CEM samples for compressive strength testing (15 samples in each group). After 7 days of incubation, compressive bond strength testing was performed using a universal testing machine. Data were statistically analyzed using the Mann–Whitney U test with a significance level of p<0.05.Results: Nine samples from group 3 were fractured during removal from the molds; the other six blocks had some cracks on their surfaces. Therefore, a compressive strength measurement was not obtainable for this group. No statistically significant difference was found between groups 1 and 2 (p>0.05).Conclusion: It can be concluded that exposure to blood does not adversely affect the compressive strength of CEM, but incorporation of blood makes the cement very brittle.

Abstract: 

In spite of major advances in the field of quality assurance in the process of collection, preparation and storage of platelets, bacterial infection following platelet transfusion remains a major problem in transfusion medicine. The present study was carried out in order to evaluate bacterial contamination of platelet concentrates collected at Tehran Regional Educational blood Transfusion Center. Materials and Methods Bacterial growth of samples of platelet concentrates was studied in blood agar, EMB and thioglycollate broth after 48 hours at 37ºC. The use of differentiation tests was made when any bacterial growth was observed. Simultaneously, the samples were also cultured in thioglycollate broth and studied for any turbidity or color change within 7 days. Any changes made the samples to be cultured in blood agar and EMB. Finally, the contamination rate and the ratio of contaminating bacteria were determined. Out of 7700 samples, three fourth (5775 samples) were taken from the cord and one fourth (1925) from both the bag and the cord. Results Out of 7700 samples of platelet concentrates studied, 14 (0.18%) were found positive for bacterial contamination. The contamination rate was estimated to be one in every 550 tested platelets (0.18%). Since in cases of blood bag contamination, the cord had been contaminated as well, there was then no difference on whether the sample was taken from the bag or cord. The bacteria identified were as follows: Staph. epidermidis (n=4), Staph. saprophyticus (n=2), Acinetobacter (n=5), Bacillus sp. (n=3). Conclusions The results show that screening platelet concentrates for bacterial contamination is necessary for blood transfusion centers and hospital blood banks.
